  • Forms required by City/County Councils, Residential Tenancies Board (RTB)
  • Forms required by the Health Service Executive (HSE)
  • Gender Recognition Certificate (GRC1 Form)
  • Health and Social Care Professions Council Registration forms
  • Land Registry Form 47 for the Property Registration Authority
  • Passport renewal/application for a child – PLEASE ENSURE that both you and your child are physically present to witness the child’s passport application, as per the guidelines of the Department of Foreign Affairs
  • Sole Guardian Affidavit Form AFF1 – PLEASE ENSURE that you bring your child’s original Birth Certificate
  • Teaching or Non-Teaching Position with a School Authority
